Add a ledger account

Register a new account so postings and exports can reference it deterministically.

Owner: bus accounts.

Add an account when you need a stable target for debits and credits that will appear in journal postings, exports, and reports. Account identity should be stable, because changing it later makes historical review and reconciliation harder.
